1. Extract folder to Documents/Sport Interactive/Football Manager 2013 or 2014/graphics/kits/put in here!
( If there is no graphics folder there, create "graphics" )
2. FM14 - click 'Load Game'....click 'FM'...click 'Preferences'...click 'Interface'....click 'Clear Cache'...click tick 'Reload skin when confirming changes' (if you first time)....click 'Reload Skin'
2b. FM15 - click 'Preferences'...click 'Interface'....click unticked 'Use caching to decrease page loading times and click tick 'Reload skin when confirming changes in preferences'.....click 'Reload Skin'.

For people asking about Wrexham: It's a problem of the default kits; in the editor Wrexham has a shirt only for 2014 season and this prevent the SS kit to show. Just tried on a 2016 save and Wrexham is showing. I think hammer will fix this in the FM15 SSKCC

Hereford United were expelled from the Vanarama Conference Premier & demoted to the Evo-Stik Southern Premier before the club was wound up in December 2014 by HM Revenue and Customs. The Hereford United Supporters Trust (HUST) have since rebuilt the club now known as Hereford FC. But it's unknown what league the team will compete in, during its inaugural season in 2015—16.
