I had a problem for a few months now (and I have noticed several people have also encountered it)... I run fm2014 on my Mac OSX and initially the facepack was working but after a few months I cleared the cache and couldn't get the facepack working.
I checked the file path was correct, made sure my settings in Preferences were correct, I even used Steam to delete Local Data and then re-installed the whole game to no avail.
I do a lot of my own Editor work, make up my own teams, create my own players so I edit my own faces quite a lot...
The problem was not my file structure, it was the fact that I had messed up my config.xml within the sortitoutsi megapack. Once I re-copied the original config.xml it was fine and my facepack reloaded with all the images appearing. I don't know what happened but I think maybe somewhere in between these updates the syntax of the config.xml is a lot stricter. I cannot even copy a line and paste it at the end in order to create a new entry. I have not worked out why.
It's difficult to spot because your config.xml can look absolutely fine but obviously there is some very strict syntax regarding new lines or whatever.
The only way i have been able to add new faces is to edit existing entries in that file. (Or of course copy the whole file and make a new config.xml deleting extra entries and over-writing the old ones).
If you are experiencing problems where your facepack will not load up and you have checked all your settings and followed all the original instructions then I would try re-downloading the original config.xml and re-load. Especially if you are the type of player that does a lot of Editor work like me.
Hope this helps because it was really confusing me for a while!
