1. Extract folder to Documents/Sport Interactive/Football Manager 2013 or 2014/graphics/kits/3Dkits/put in here!
( If there is no graphics folder there, create "graphics" )
2. FM13 - click 'Preferences'...click 'Interface'....click unticked 'Use caching to decrease page loading times and click tick 'Reload skin when confirming changes in preferences'.....click 'Reload Skin'.
2a. FM14 - click 'Load Game'....click 'FM'...click 'Preferences'...click 'Interface'....click 'Clear Cache'...click tick 'Reload skin when confirming changes' (if you first time)....click 'Reload Skin'

I found the problem in the config.xml - Blackburn third kit - missing ">"

Forest 3rd kit is not in this pack.
3D kits are used in the match engine so you will see them whilst watching the match in 3D, they wil show these custom versions that have been created to look like the real kits.
New stuff is being created check the wip, just be patient as they will be released once someone has the time to created them.

Sorry I thought you were talking about kits generally.
The forest third is not in the game at all. SI need to add it in order for us to write an image over the default kit_textures they provide. At the moment they don't provide it hence why it will never even be an option.
A later update may provide it but you can't add a team a third kit without editing your game database with the editor prior to starting a new game. I do not know how this is done but I know of a few people who do it.
SWFC's third kit is in this 3D pack that was created but unless the game has a third kit slot for a team (for SWFC it currently does not) then no amount of coding or kitpack will allow it to be seen. Try the editor pre-game until a later update.
