1. Extract folder to Documents/Sport Interactive/Football Manager 20##/graphics/kits/put in here!
( If there is no graphics folder there, create "graphics" )
2. FM14 - click 'Load Game'....click 'FM'...click 'Preferences'...click 'Interface'....click 'Clear Cache'...click tick 'Reload skin when confirming changes' (if you first time)....click 'Reload Skin'
2b. FM15 - click 'Preferences'...click 'Interface'....click unticked 'Use caching to decrease page loading times and click tick 'Reload skin when confirming changes in preferences'.....click 'Reload Skin'.

Cardiff Kits issues...
go C:\Program Files (x86)\Steam\SteamApps\common\Football Manager 2015\data\graphics\pictures\kits\2d\outfield\front\eng
you see Cardiff defaults there...delete them....then reload skin on FM15 Cardiff SS kits back on.

I wanted to break down how to fix the Cardiff kit error or any team kit error for people that use Mac's as this doesn't seem to be clear here.
Ok, so hamer gave you all the important steps with one key step missing - so lets go through it.
1) Open Football Manager Resource Archiver 2015 (download from tools section of library in Steam)
2) Use 'Extract Archive' function to extract graphics.fmf from here: User > Library > Application Support > Steam > SteamApps > common > Football Manager 2015 > data
3) Extract the graphics.fmf in the same folder where its located (link above) and then go into your new extracted folder
User > Library > Application Support > Steam > SteamApps > common > Football Manager 2015 > data > graphics > pictures > kits >2d > outfield > front
4) Once you are in the "front" folder DELETE EVERYTHING, all the kits within there.
5) Repeat the steps above for the same graphics.fmf files that are in the updates folders: User > Library > Application Support > Steam > SteamApps > common > Football Manager 2015 > data > updates
6) THE MOST IMPORTANT STEP - DELETE THE OLD graphics.fmf files!!!!!!
7) Start a new game and reload the skin - Thank Me Later!!

I am in the same boat. I either get all blank kits for the Championship files, or the default in game ones (even when they have been deleted from the hard coded files)
The annoying this is, the League One and League Two files work perfectly, without even editing the fmf file.
I have no idea why the Championship one does not work.

Same as. It's the only graphics file that doesn't work for me on Mac.
I can live it, but if there was a solution...it would be grand!
