Anyone having substitute problems?

Hi, last few games I have subbed a player and confirmed and when I go back to game he is still playing and in a different position. Also I have subbed a player and a different player has come on when I go back to the game, the best or worst example was when I subbed off the centre forward and the game put my reserve keeper up front! This has happened several times in the last few days up till then no problems and I have restarted the games and subbed in different ways, any help or comments would be useful, thanks.
