Not sure to be completely honest. The BepInEx logs may show you a unique ID which you can set as the filename for your online manager, assuming it's just using the manager.png now.

All of the files/folders in the download need to be extracted to your game directory, not just the BepInEx folder.

Not from me, no, it's not something I've looked into at all. It's a licensing thing in the game, I would expect this to be resolved in the next game update (or whenever the World Cup license is added).
