I remember somebody posted some football boots last year and I have been making my own so I decided to share with you guys. 
To install copy boots.png and boots.dds from folder of boots you plan to use into:
Players Install : Steam\steamapps\common\football manager 2013\data\sigfx\players\outfield
goalkeeper Install : Steam\steamapps\common\football manager 2013\data\sigfx\players\goalkeeper

the game doesnt allow players with different boots, must all have the same ones unfortunately

No template I just make new ones every now again using other pictures as a guide to know here to place the boots and what size they should be.
Just cut a good quality pic of boots on photoshop, resize and position them as a layer on an old boots.png, use color overlay on the old boots.png that matches the new ones and the save as a dds file using dxt5 (to do this you need the photoshop plugin).
Have a try and you will figure it out along the way, not too difficult.
