I downloaded the whole 5.08 icons megapack as well as the 5.08 update pack on its own, and both were in the correct size

I've downloaded the update. Do I move the updated file contents only or along with its xml file into my base MP?

I download icons megapack 6.0 and extract to Documents/Football Manager 2014/graphics/faces/Icons Pack 6.0.
Into the game I view only large photo but no this small photo.
What's the correct path to insert?

I put large photo in C:/Users/User/Documents/Sports Interactive/Football Manager 2014/graphics/faces and work perfectly.
Small photo in C:/Users/User/Documents/Sports Interactive/Football Manager 2014/graphics/faces/Icons pack 6.0 or C:/Users/User/Documents/Sports Interactive/Football Manager 2014/graphics/pictures/Icons pack 6.0 and doesn't work!

Have you tried reloading the skin? As long as it is within graphics, it should work, irrespective of the folder name. Also, are you sure you download the whole pack (i.e. with 135,000+ images) and not the latest update pack only?

I've altered some "new players" and given them faces, but I can't get the icons to work.
Is it the same process using fm.xml that you would use for creating a profile picture?

Yes, but then you just need to amend the config file manually, since fmXML only has the facility to create the main player faces and not the icons.
All you need to do is create a new config file with fmXML, then open it with notepad, press CTRL + H to find all entries of
