You didn't use fmf file from pack when starting new career.
If you play old career, then you have to use in-game editor to erase all shirts in kits & color section that have a competition assigned to them.

It's the same file but occasionaly one might be more up to date than the other, so it's always good to check the date of the files.
At this moment, the one from SS pack is more recent and should be used.

You're correct, I forgot to add Burnley third in editor. I'll reupload the file tomorrow morning.
You can also add shirt in kits & colors section in in-game editor so you don't have to wait
