Surely the first one is:

Offside at the moment the initial shot was taken as there were not two defending players between the attacker and the goal

Or am I reading it wrong?

Also, I thought they flipped a coin to see which end the penalties were taken or is that just to pick who goes first?

Nope, because he's not interfering with play until he receives the ball from the goalkeepers touch at which point he becomes active and offside.

Yeah, but the relevant bit in the laws of the game to this is a section that says something along the lines of 'being in an offside position is not an infringement in itself, but becomes so once an advantage has been gained from being in that position'.

I'll find the exact wording in a sec.

EDIT:

It's the first sentence of the section of offside

it is not an offence in itself to be in an offside position

which is elaborated on later:

A player in an offside position is only penalised if, at the moment the ball touches or is played by his teammate, he is, in the opinion of the referee, involved in active play by:

interfering with play
interfering with an opponent
gaining an advantage by being in that position

are two I definetly didn't get, plus I think the head the ball on a penalty, number of players

plus number 10, offside in centre of the pitch. I thought you can't be offside from throw in ?
