Winning the Serie D playoff does not grant direct promotion. Winning the playoff final only serves to position the club at the top of a priority table known as the Graduatoria di Ripescaggio (relegation playoff ranking).
Vacancies in Serie C: If any Serie C (third division) club goes bankrupt, fails to meet financial requirements, or fails to register before the end of the season, a vacancy opens up.
Relegation Playoff (Ripescaggio): The Italian Football Federation (FIGC) uses the list of Serie D playoff winners to invite those clubs to fill the vacant places in Serie C.
If no Serie C club goes bankrupt or is administratively relegated at the end of the season, the relegation playoff is not activated, and the playoff winner remains in Serie D for another year.

No. As it's stated in the opening post, the mod includes divisions up to Promozione.

You have to use that file in the Skin Builder app (so you have to download and install it), and launch it from there. That's all I know.
