This file will need to be re-applied if/when patches/updates are released to ensure full compatibility. This is due to the fact that some files which are removed when installing this file are automatically re-installed by Steam when official patches come out.
The sortitoutsi real names fix changes the instances of fake names in Football Manager 15 to the proper, real life names for awards, cities, stadiums and competitions, in cases where the actual name is not licensed. It will also fix the fact that the German National team doesn't call up real players. These are fully save-game compatible.
There is also a club name changes file, which changes the names of clubs in a way which ensures that any nicknames entered in the default database are retained. This file includes the proper names for German, Italian and French clubs to circumvent licensing restrictions. It also adds real-life media and journalists, with 26 media files added to encompass all major footballing nations and continents. Finally, it includes a small file which corrects the 3-Letter competition names to correspond with any changes made in the lnc files. For all these files to work, however, you will need to start a new game.
Magrippinho very helpfully created a YouTube video on how this file should be installed. Some of the contents are different - but the procedure is identical, although of course you use the equivalent folder for FM15.
Or, you can follow the below instructions to install v2.0 of this file (if you are installing a previous version please use the 1500 folder instead of the 1530 folder:

Step 3: Copy the edt and lnc folder from this download into the folder mentioned in step 1.

Step 7: Paste the circled folder directly into the folder mentioned in step 5.
Step 8: This should be what your editor data folder looks like. There may also be a config.xml file in this folder and can be safely retained; it is generated by FM every time the game starts.
To make sure the files have been applied correctly, go to the Champions Cup and EURO Vase in your saved game, and they should be renamed to Champions League and Europa League respectively. If you are in the middle of a save-game, German national teams will be populated in the next instance that players are called up, and not instantly. Club names (including the German ones) are only changed if you apply the editor data files before starting a new game. For a workaround if you've already started a new save-game, then please read the opening post of this thread.

The reserves have been changed from, eg, Stoke City Reserves to Stoke City II - as a result of this, the logos assigned to the senior team don't transfer down to the reserves anymore, but they do to the Under 19s, despite being listed as Stoke City III

That is completely bizarre and has nothing to do with this file. I suspect you have downloaded a separate editor data file which includes the changes you have just described. Can I have a look at your editor data folder so I can perhaps pinpoint which one it is?

Nailed the league name, but the clubs aspect remains - did you change the reserves to "II"?

Engelse is Dutch for English (at least according to what Google seems to be telling me), so I think that the league name aspect is either from a Dutch rip-off of this file (in which case applying these files means the changes are save-game compatible) or from a Dutch author of an editor data file who changed the competition names into the Dutch language using the pre-game editor (in which case you need to start a new game to have the correct "English" names, I'm afraid). You seem to have solved this issue, at least.
As for the reserves part, it is only possible, as far as I can tell, to edit clubs' reserve or youth team names in the manner you are describing, by changing the league system to incorporate II teams. As far as I am aware, the only countries which have reserve teams being named as II teams are Germany and France - which would lead me to think that you have downloaded either a German or French file doing just that, or one made by a German or French site or user which modifies the league setup in that way. The only way this issue can be solved is by starting a new game, with the guilty file being removed.
Either way, I'm afraid that your issue has nothing to do with the files from the download on this page but hope the above helps you pinpoint which file(s) caused the issue.
While I cannot tell users how to play their game and what files to download, I would strongly suggest gaining some familiarity with the files one downloads before using them to ensure you are aware of what these files actually do

The club name changes file is an editor data files and editor data files don't work for FMC.

For licensing reasons, Japan-based players cannot be in the database. This file does not include them and has never purported yo do so. In order to have them in your game, I would suggest downloading widehawk's excellent Japanese league add-on complete with full squads. You can locate it elsewhere online via Google

Due to Portuguese licensing restrictions which are hard-coded, the long and short club names will be identical, but they are already in this file as Porto, Benfica and Sporting CP respectively...
Man I thought using fake names could circumvent the licensing restrictions...sigh, thanks anyways mate
EDIT: I didn't realize we had to overwrite both the 1500 and 1530 folders :p, my mistake, it works fine now
