I think the biggest bug of all, with far-reaching ramifications is the duplicate regen IDs. Regens frequently assume IDs from players that are already in the database. The most noticeable impact is regens getting the same player face as the ID they've been assigned, but I'm quite concerned how this will affect things under the hood in a long-term save. This has the potential to be severely game-breaking and SI have barely acknowledged it.

Yeah, we did try putting the same .fmf in the client's editor data. As you say, it shouldn't make a difference but we thought it was worth trying anyway. Unfortunately the problem persisted.
The issue seems to happen on the news items and in search. One particular example, the client joined a club and the media source reporting it was listed as CNN Turk, which I thought was odd as it was a lower league club in England. When I clicked on CNN Turk from the media page, the URL was actually for BBC Radio 5 Live and all the journalists seem to verify that it should indeed have been BBC Radio 5 Live. If I searched for “BBC Radio 5 Live” in the search bar, it'd return CNN Turk in the results. If I searched for “CNN Turk”, it'd return “Tribuna”. This is just one example, but this is seemingly the same for most media outlets.
All media outlets seem to be present, but the names seem to be mismatched to what is actually showing on the media outlet page.

Edit: Just to reiterate, this is from the perspective of the client. All seems fine on the host end. We have tried multiple hosts.
Edit 2: The only other difference is that one of us using the Windows Store version and the other is using the Steam edition. Not sure why it'd make a difference, but worth mentioning.

Any idea why the media outlet names could be mixing up in a multiplayer game? Have tried it without any other database mods and the issue is always the same. The media outlets seem to work as intended for the host, but any clients will see media outlets with another media outlet's name i.e. CNN TURK is called Tribuna, Radio 5 Live is called CNN TURK.
